In the 10th and 11th centuries, Italy's population grew dramatically. Personal and legal confusion began to arise around identities and many found that they could no longer identify themselves merely with a single name. Hereditary surnames were the solution to this problem. Occupational names, like Azzaro, were derived from words identifying the bearer by his profession. Azzaro was a name for a person who made battle axes as it is derived from the Italian word azza, which means battle-axe.

The surname Azzaro was first found in at Novara, capital city of the province of Novara in the Piedmont region in northwest Italy in 1312, with Pietro Azario, a count and wealthy lawyer, registered in that year.
